HPISD and the HP Sports Medicine Team require that students in 7th-12th grade participating in UIL sponsored athletic activities update their physical every year. The physical must be completed using the
UIL Pre-Participation Physical form and the exam may be completed no earlier than May of the previous school year.
Student-athletes will not be allowed to try-out, practice, or compete without having a physical on file with the HP Sports Medicine Department along with all of the required online forms.
FORMS NECESSARY PRIOR TO PARTICIPATION – All Grades 7-12
• Pre-Participation Physical Form – “Date of Exam” on page 2 must be listed as on or after May 1st of previous school year. (example May 1st, 2026, for 2026-27 School year)

UIL Pre-Participation Physical form needs to be completed and signed by a physician before being turned into your Coach, an Athletic Trainer, or the Athletic Office at Highlander Stadium. The
"date of exam" field must be May 1st of the previous school year or later. You may turn in hard copies or email PDF copies, but photos will not be accepted. It is recommended you keep a copy for your records.
You will need to complete this process separately for each individual student, but only once per school year.
